Severe pain during ovulation: what to know

Ovulation pain, or mittelschmerz, may feel like a brief sharp sensation or an ache on one side of the lower abdomen. It can occur when the follicle stretches the ovary or releases fluid that irritates nearby tissue. Severe pain is not automatically part of normal ovulation. An ovarian cyst can present with one-sided pelvic pain or pressure, and endometriosis can present with recurring cycle-related pelvic pain. Pain may also come from infection, the bowel, urinary tract, muscles, or a pregnancy-related problem. Location and timing help a clinician ask better questions but cannot name the cause.

Note the day of your cycle, side, sensation, duration, bleeding, discharge, temperature, nausea, and bowel or urinary changes. If the pain is mild and settling, rest, gentle heat, and light activity may be comfortable. Make an appointment when pain is severe, repeats, lasts longer than your usual episodes, or changes your ability to work, sleep, exercise, or have sex. Mention contraception and pregnancy possibility. A clinician may recommend an examination or imaging based on the full pattern.

Get urgent medical help for sudden severe or rapidly worsening pain, fainting, severe dizziness, heavy vaginal bleeding, fever, repeated vomiting, a rigid abdomen, or possible pregnancy. Seek prompt advice for new pain with unusual discharge, painful urination, or inability to keep fluids down. These symptoms need evaluation regardless of where you are in your cycle.
