Severe ovulation pain on the left side: what to know

Ovulation pain, also called mittelschmerz, can occur on the side releasing an egg and may feel like a pinch, ache, or cramp. The location alone cannot confirm that ovulation is the cause. Left-sided pelvic pain can also come from the ovary, bowel, urinary tract, muscles, or reproductive organs. An ovarian cyst can present with one-sided pelvic pain, while endometriosis may present with recurring pain linked to the menstrual cycle. Notice whether the pain moves, follows meals or bowel movements, comes with urinary changes, or repeats at a similar point in each cycle.

Write down the start time, exact location, intensity, bleeding, discharge, bowel symptoms, urinary symptoms, and any chance of pregnancy. Gentle heat and rest may help if the discomfort is mild and settling. Use only pain medicine that is appropriate for your health and follow its label or a clinician's advice. Book an appointment if pain is severe, keeps returning, lasts longer than expected, or disrupts work, sleep, or movement. Assessment may include questions, an abdominal or pelvic examination, and imaging when indicated.

Go to urgent care for sudden or worsening pain, faintness, weakness, heavy bleeding, fever, vomiting, a swollen or rigid abdomen, or pain with a positive pregnancy test or possible pregnancy. These features can signal a problem that cannot be safely sorted out by cycle timing. If symptoms are uncomfortable but stable, contact a healthcare professional promptly rather than repeatedly self-treating.

Ask whether the left-sided timing is consistent with ovulation, what other causes fit your symptoms, whether a cyst or endometriosis needs evaluation, and what changes should send you to emergency care. Mention any previous pelvic surgery or known conditions.
