Severe headache when reaching climax: what to know

Some people develop a throbbing pain that builds with sexual excitement or peaks abruptly at orgasm. Muscle tension, exertion, migraine, dehydration, or anxiety may contribute. However, a sudden explosive headache can also signal bleeding or another problem involving the brain or its blood vessels. The symptom alone cannot distinguish these causes.

Stop sexual activity, rest in a quiet place, and avoid driving if the pain is severe or your vision feels abnormal. Note when it began, how quickly it reached maximum intensity, where it is located, and whether vomiting, neck pain, weakness, or confusion occurred. Do not restart activity until a clinician has assessed a first episode. Hydration may help when you have been sweating or drinking too little, but it should not delay care.

Seek emergency care immediately for a headache that reaches maximum intensity within moments, follows a first episode, or comes with fainting, seizure, confusion, fever, stiff neck, repeated vomiting, weakness, numbness, trouble speaking, or vision loss. Also seek urgent assessment if it follows head injury or keeps returning with increasing severity. A clinician may examine you and decide whether brain imaging or other tests are needed.

Ask whether the timing and pattern fit a primary sex-related headache, whether migraine or blood-vessel causes need checking, and which symptoms should trigger emergency help if the pain returns.
