Severe Headache and Stiff Neck: What to Know

Neck stiffness may make it difficult or painful to bend your chin towards your chest. Alongside a severe headache, it can occur with infections affecting the brain or its surrounding tissues, bleeding around the brain, or a severe migraine. Fever, light sensitivity, repeated vomiting, a new rash, drowsiness, or confusion increase concern.

Headache and neck discomfort can also accompany viral illnesses, dehydration, heat exhaustion, or sinus symptoms. These possibilities do not safely explain away a sudden, unusually intense headache or marked stiffness. A clinician may need to examine you promptly and decide whether tests or hospital care are needed.

If the pain is severe and the neck is stiff, arrange urgent medical assessment now. Do not drive yourself if you feel faint, confused, weak, or unusually sleepy. Note when the headache started, whether it reached maximum intensity quickly, and any fever, rash, injury, new medicine, or recent illness.

While arranging help, rest somewhere cool and quiet, avoid alcohol, and take small sips of fluid if you can swallow normally. Do not force your neck to move or delay assessment by repeatedly treating the pain at home.

Call emergency services immediately for a sudden thunderclap headache, fainting, seizure, confusion, difficulty speaking, new weakness or numbness, trouble walking, breathing difficulty, or repeated vomiting. Seek emergency help as well for fever with a stiff neck, a rash that does not fade when pressed, or rapidly worsening symptoms.

Do not wait for a rash or fever to appear before getting help. If symptoms are severe but none of these warning signs are present, same-day urgent assessment is still appropriate.

Ask what might be causing the headache and stiffness, which examination or tests are needed, and which changes should prompt immediate return. Tell the clinician about recent infections, travel, insect bites, head or neck injury, pregnancy, immune problems, and every medicine or supplement you use.
