Severe hamstring pull: what to know

Short answer
A severe hamstring pull should be assessed promptly, protected from further strain and rehabilitated with progressive strength work.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical location
- Back of the thigh, sometimes close to the buttock or knee
- Key warning sign
- Weakness, bruising or inability to walk normally
What may be injured
The hamstrings are the muscles at the back of the thigh. A forceful sprint, sudden stretch or poorly controlled landing can strain them, causing sudden pain, tenderness and weakness. Bruising or swelling may appear later, and sitting can be uncomfortable when the upper tendon is involved. Thigh pain can have causes outside the hamstrings, so a clinician may check the hip, back, nerves and muscle strength before deciding how much loading is appropriate.
Protect the area and rebuild capacity
Stop sprinting, jumping and deep stretching after the injury. Walk only as comfortably as possible, avoiding a pronounced limp. Use a wrapped cold pack for short periods if it eases symptoms, and avoid forceful massage early on. Once examined, exercise therapy can restore range, control and strength through suitable stages. A return-to-sport programme should include jogging, acceleration and sport-specific drills before competition. PRP injection may be discussed in selected cases, but it is not a substitute for diagnosis and rehabilitation.
When to seek care
Seek prompt care if you felt a pop, cannot walk normally, have marked bruising, a visible dip, major swelling or substantial weakness. Urgent help is needed for a cold or discoloured leg, spreading numbness, severe unrelenting pain, or symptoms after major trauma. Review is also sensible when pain continues or repeatedly returns as activity increases.
Questions for your doctor
Ask which part of the hamstring is affected, whether the tendon is involved, when strengthening can start, how to judge readiness for running, and whether any procedure has a clear role for your injury.

Questions people ask
Should you stretch a pulled hamstring immediately?
Forceful stretching can aggravate an acute injury; use comfortable movement and obtain an assessment first.
What determines return to sport?
Pain-free movement, restored strength and controlled running tasks matter more than a preset date.
