Severe fatigue causes: what to know and when to seek care

Short answer
Severe fatigue can reflect poor sleep, dehydration, anaemia, infection, emotional strain, or an underlying medical condition, so persistent or sudden exhaustion deserves assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What severe fatigue may mean
Fatigue is more than feeling ready for bed: it can make ordinary tasks, concentration and movement feel unusually difficult. Recent illness, disrupted sleep, heavy workload and low mood can all reduce energy. Other possibilities include Anaemia, Coeliac Disease, Chronic Kidney Disease, COVID-19, Dehydration or Burnout. Alcohol Use Disorder and Chronic Fatigue Syndrome may also be relevant in particular patterns of symptoms. The cause cannot be identified from tiredness alone; the timing, associated symptoms and medicines matter.
What to do next
Record when the fatigue began, whether it is constant, how sleep feels, and what makes it better or worse. Note fever, breathlessness, palpitations, weight change, thirst, pain, bowel changes and mood symptoms. Eat regular balanced meals, drink fluids, and keep activity gentle while you feel unwell. Arrange a primary-care assessment if fatigue persists, interferes with daily life, or returns repeatedly; an examination and selected tests may help identify the cause.
When to seek urgent care
Seek urgent medical help for fatigue with chest pain, severe breathlessness, fainting, new confusion, one-sided weakness, severe dehydration, black or bloody stools, or a rapidly worsening illness. Prompt assessment is also sensible when exhaustion follows significant bleeding, pregnancy-related concerns, or an inability to stay awake. Do not drive if you feel faint, confused or too weak to travel safely.
Questions for your doctor
Could my sleep, medicines, diet, mood or recent infection explain this fatigue? Which symptoms would change the urgency? Do I need blood or urine tests? What level of activity and fluid intake is appropriate while the cause is being assessed?
