Severe bronchiolitis treatment: what to know

Short answer
Severe bronchiolitis treatment focuses on supporting breathing, oxygen levels, hydration, and feeding while the child recovers.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What severe bronchiolitis means
Bronchiolitis is an infection that causes swelling and mucus in the smallest airways, most often in babies and young children. Severe illness can make breathing visibly harder and may reduce a child’s ability to drink or breastfeed. Clinicians assess breathing effort, oxygen level, alertness, wet nappies, and feeding. A cough, fever, or wheezing can accompany bronchiolitis, but severity is judged by the child’s overall breathing and hydration rather than one symptom alone.
How treatment is provided
In a healthcare setting, treatment may include oxygen, gentle clearing of nasal secretions, and fluids when drinking is not enough. A nebuliser may be considered in selected situations, but medicines are not automatically helpful for every child with bronchiolitis. Keep the child upright while awake, offer small frequent feeds, and monitor wet nappies. Avoid cigarette smoke and do not give cough medicines unless a clinician specifically recommends them. A child health consultation can assess changes and decide whether observation or hospital care is needed.
When to seek urgent care
Seek urgent medical help if the child is struggling to breathe, has pauses in breathing, develops blue or grey lips, is unusually floppy or difficult to wake, or cannot keep fluids down. Reduced wet nappies, very poor feeding, or rapidly worsening breathing also need prompt assessment. Babies who were born prematurely or have heart, lung, or immune problems may need earlier review. Call emergency services if breathing stops or the child becomes unresponsive.
Questions for your doctor
Ask which breathing changes should trigger immediate review, how often to offer feeds, how to check hydration, and whether oxygen or fluids are needed. Confirm which medicines to avoid and when follow-up should happen.

Questions people ask
Will antibiotics treat severe bronchiolitis?
Bronchiolitis is usually caused by a virus, so antibiotics do not treat the bronchiolitis itself. A clinician may prescribe them only if a separate bacterial infection is suspected.
Can severe bronchiolitis be managed at home?
Some children can be monitored at home, but a child with difficult breathing, poor feeding, or dehydration needs urgent clinical assessment.
