SASD Bursitis: What to Know
Updated

Short answer
SASD bursitis is irritation of the fluid-filled cushion beneath the shoulder roof, causing pain with lifting the arm and lying on that side.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Affected area
- Bursa beneath the shoulder roof
- Typical trigger
- Overhead reaching or pressure while lying on the shoulder
- Helpful principle
- Keep gentle movement while reducing irritation
What it means
The subacromial-subdeltoid bursa helps tendons glide around the shoulder. Repeated overhead work, a sudden increase in activity, a fall, or shoulder mechanics can irritate it. Pain may sit on the outer upper arm, worsen when reaching overhead or behind your back, and disturb sleep on the affected side. Bursitis can overlap with rotator cuff irritation or other shoulder conditions, so the label is not confirmed by pain alone. Examination may include strength and movement checks; imaging is considered when the diagnosis is uncertain or recovery is atypical.
What you can do
Temporarily reduce repeated overhead reaching and keep loads close to the body. Continue gentle, pain-limited shoulder movement so the joint does not become stiff. Ice or heat may be useful according to which feels better. A clinician can discuss anti-inflammatory medication if it is appropriate for you, considering stomach, kidney, heart, pregnancy, and other risks. Physiotherapy may address movement control and strength. A cortisone injection is sometimes considered when pain remains limiting; it is a clinical decision, not a substitute for diagnosis.
When to seek care
Book an assessment if pain persists, arm movement is becoming more restricted, or symptoms began after an injury. Seek urgent care for a hot, red, rapidly swollen shoulder with fever, sudden inability to lift the arm, chest pain, breathlessness, or new weakness or numbness. Joint aspiration may be discussed when infection, bleeding, or another fluid-related problem needs investigation.
Questions for your doctor
Could another shoulder problem be causing this pain? Which activities should I modify? Is medication, physiotherapy, injection, or aspiration appropriate for my symptoms and health history?

Questions people ask
Can SASD bursitis cause pain at night?
Yes. Pressure from lying on the affected side and pain with repositioning can disturb sleep.
Is an injection always needed?
No. Many treatment plans begin with activity adjustment, movement, and rehabilitation; an injection depends on the assessment.
