What should you know about an S3 gallop and heart failure?

The S3 is heard shortly after the heart's main pumping sound, during the phase when the ventricles fill. In an adult, it can suggest that blood is entering a ventricle under increased pressure or that the ventricle is enlarged or less efficient. It is a physical finding, not a diagnosis by itself.

Heart Failure may present with breathlessness, reduced exercise tolerance, fatigue, a cough or swollen ankles. A clinician may also consider Hypertension, Atrial Fibrillation, valve disease, previous heart muscle injury and kidney problems. Examination is commonly combined with an ECG, blood tests and an echocardiogram to understand the cause and pumping function.

Arrange the recommended assessment even if you feel reasonably well. Bring a list of medicines and note breathlessness when lying down, waking at night, changes in weight, ankle swelling, palpitations and activity tolerance.
