Short answer

Right-side ovulation pain is usually felt low in the right pelvis, although it may spread toward the groin, back, or upper thigh.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Where the pain may be

The discomfort may sit beside the pelvic bone, deep behind the lower abdomen, or close to the hip crease. Some people describe a small, clearly localised twinge; others feel a dull ache that radiates toward the groin, lower back, or upper thigh. It may appear around the middle of the cycle and settle without treatment. The exact location is not enough to identify the cause.

Endometriosis can present with pelvic pain and may affect nearby areas, while Ovarian Cysts can cause pressure or one-sided pain. Bowel, bladder, muscle, and pregnancy-related problems can also overlap with this location, so a changed or persistent pattern deserves evaluation.

How to monitor it

Mark the painful spot and note whether movement, urination, bowel movements, sex, or eating changes the sensation. Record the cycle day, intensity, duration, bleeding, discharge, nausea, and fever. A warm pack, comfortable movement, hydration, and suitable over-the-counter pain relief may help mild symptoms; follow the label and ask a pharmacist if you might be pregnant.

Arrange a routine appointment if the pain returns often, persists beyond the expected mid-cycle window, becomes more intense, or limits normal activity. A clinician can decide whether examination, pregnancy testing, or imaging is useful.

When location becomes urgent

Seek urgent care for sudden severe right-sided pain, fainting, heavy bleeding, fever, repeated vomiting, severe weakness, or a swollen, rigid, or very tender abdomen. One-sided pelvic pain with possible pregnancy should be assessed promptly, particularly when dizziness or bleeding is present.

Questions for your doctor

Ask whether the location and cycle timing fit ovulation pain, what nearby organs could produce similar symptoms, and whether testing is needed. Ask for clear instructions about monitoring, suitable pain relief, and the warning signs that should take you to urgent care.
