What should you know about reverse freckles?

Freckles, also called ephelides, are small flat brown or tan spots caused by increased pigment activity. They commonly become more noticeable after sun exposure and lighter during periods with less ultraviolet exposure. Their colour and shape can vary, but a typical freckle is flat and fairly uniform.

A new or changing pigmented spot should not automatically be labelled a freckle. A clinician may use close examination or dermoscopy to compare its border, colour, and structure with surrounding skin before discussing cosmetic treatment.

Use broad-spectrum sunscreen on exposed skin, reapply as directed outdoors, and combine it with shade, protective clothing, and a hat. These habits help limit darkening and protect the surrounding skin. Do not pick, burn, or apply unregulated lightening mixtures, which can cause irritation or uneven pigment.

The right choice depends on the diagnosis, skin tone, recent tanning, medicines, and risk of temporary or longer-lasting colour change. Ask how the skin will be protected before and after treatment and what signs of irritation need review.

Arrange a prompt skin examination if a spot changes in size, shape, border, or colour, becomes raised, bleeds, crusts, itches persistently, or develops a sore that does not heal. Have the area assessed before a light-based procedure so a clinician can confirm that it is suitable for cosmetic treatment.
