What should you know about retrosternal multinodular goitre?

A retrosternal multinodular goitre is a thyroid enlargement with several nodules that extends behind the breastbone. Its impact depends on the amount of chest extension, the airway space available, thyroid hormone production and whether symptoms are developing.

Multinodular means the thyroid has multiple areas with different tissue structure; retrosternal means part of it lies below the neck. The gland may work normally, release too much hormone, or produce too little. Pressure can cause throat fullness, cough, swallowing difficulty, voice change or breathlessness, particularly when lying down. Imaging is useful because the chest portion cannot be assessed by looking at the neck alone.

Arrange a GP consultation for a new swelling, changing symptoms or a known goitre that has not been reviewed. Assessment commonly includes a neck examination, a thyroid function test and imaging chosen to show the nodules and their extension. Management may be observation, treatment of abnormal hormone levels, or specialist planning if compression, growth or an uncertain nodule needs attention.

Seek emergency care for severe or sudden breathlessness, inability to swallow saliva, collapse, blue lips or rapidly enlarging neck swelling. Contact a clinician promptly for new noisy breathing, worsening symptoms when lying flat, persistent hoarseness or strong palpitations. Keep a record of symptom changes to help the assessment.

Ask how many nodules need follow-up, whether any have concerning features, how far the goitre extends, and whether the airway is narrowed. Clarify your thyroid results, imaging plan, review timing and symptoms that should prompt urgent help.
