What should you know about retrosternal goitre sign?

The thyroid sits at the front of the lower neck. When part of an enlarged thyroid grows downwards into the upper chest, the swelling may be less obvious from outside the neck. A clinician may suspect this from symptoms, an examination or imaging.

A retrosternal extension can cause a feeling of tightness in the neck or upper chest, difficulty swallowing, noisy breathing, breathlessness when lying down, or a change in the voice. Some people have no pressure symptoms, and the thyroid may still need assessment because its position can make examination more difficult.

A goitre can occur with normal thyroid hormone production, an overactive thyroid, or an underactive thyroid. The sign itself does not establish the hormone state or explain whether a thyroid area is benign or needs further investigation. Blood tests and imaging help distinguish these possibilities.

Arrange a GP consultation if you notice a new or enlarging neck swelling, persistent swallowing difficulty, breathlessness, voice change, or pressure that is worse when lying down. Tell the clinician when it began, whether it is changing, and whether symptoms affect eating, sleep or activity.

Assessment may include feeling the neck, checking breathing and swallowing, and arranging a thyroid function test. Ultrasound can assess the part of the thyroid in the neck; cross-sectional imaging may be considered when the lower extent behind the breastbone needs clearer definition. Do not change thyroid medicine without medical advice.

Seek urgent medical help if breathing becomes difficult, noisy or rapidly worse, or if you cannot swallow saliva. Sudden worsening of neck or chest pressure, blue or grey lips, fainting, or severe voice change also needs immediate attention.

While waiting for help, stay upright if that makes breathing easier and avoid food or drink if swallowing is unsafe. A stable neck swelling still deserves a planned appointment, even when it causes little discomfort.

You could ask: Does the thyroid extend into the chest? Do my thyroid hormone results show normal, high or low activity? Which scan is appropriate, and what symptoms should prompt urgent review? If treatment is suggested, how will it address pressure on breathing or swallowing?
