Retrocalcaneal bursitis: what to know

How the structures involved in Bursitis differ from normal

Short answer

Retrocalcaneal bursitis is inflammation of the bursa between the heel bone and Achilles tendon, causing pain and tenderness behind the heel.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Anatomy
Bursa between the calcaneus and Achilles tendon
Often aggravated by
Back-of-heel pressure and repeated ankle loading

What is happening

A bursa is a small fluid-filled sac that helps tissues glide. At the back of the ankle, repeated rubbing or compression can irritate the retrocalcaneal bursa. Pain often sits deep behind the heel, with tenderness when shoes or the Achilles area are pressed. Walking uphill, climbing stairs, running and ankle movement may aggravate it. A healthcare professional checks the heel, tendon, skin and range of motion because similar symptoms can come from Achilles tendon injury, a heel-bone problem or infection. Swelling around the joint can be one part of the assessment.

Practical treatment steps

Temporarily lower impact activity and avoid shoes that press on the back of the heel. Supportive footwear and a gradual return to loading may help. Cold packs protected by a cloth can reduce discomfort. Ask a pharmacist or clinician about anti-inflammatory medication, especially if you have stomach, kidney, heart or pregnancy-related concerns. Physiotherapy can work on calf strength, flexibility and ankle mechanics. Persistent symptoms may need examination and imaging; fluid aspiration or an injection is considered only after the cause and safety have been reviewed.

When to get medical help

Book an appointment when pain persists, interferes with normal walking or returns whenever activity resumes. Seek prompt care if the heel becomes hot and red, swelling grows quickly, fluid drains from the skin, or fever develops. Get urgent help after a significant injury if you cannot bear weight or push the foot downward. New calf swelling, chest pain or breathlessness also needs urgent assessment.

Useful questions

Ask whether the bursa, Achilles tendon or heel bone is responsible; whether imaging is needed; which footwear and exercises are suitable; and how to increase activity without provoking symptoms. Clarify the expected follow-up plan and the signs that require earlier review.

Questions people ask

Where does this bursitis hurt?

It usually causes tenderness or deeper pain behind the heel, close to the Achilles tendon, rather than under the foot.

Should I stretch the calf?

Gentle stretching may be appropriate, but forcing ankle motion into pain can increase irritation. Get individualized guidance if symptoms are significant.
