What should you know about restless legs at night causes?

Short answer
Restless legs at night can have several causes, including iron deficiency, nerve symptoms, kidney disease, medicines, or restless legs syndrome.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Pattern to note
- Rest, movement, time of night, and one-sided or two-sided symptoms
- Testing principle
- Investigations should be chosen from the symptom pattern and medical history
Possible contributors
Restless legs syndrome commonly presents with an urge to move the legs and unpleasant crawling, pulling, or tingling sensations that begin or worsen during rest. The cause may be unclear, or there may be a family pattern. Iron deficiency anaemia, chronic kidney disease, pregnancy, and some medicines can also be relevant.
Peripheral neuropathy may cause burning, pins and needles, pain, or reduced sensation. Those symptoms can occur at night but do not necessarily improve with movement. The timing of symptoms alone cannot identify the cause, so note the full pattern rather than focusing on one possibility.
How to investigate the cause
Write down when symptoms start, whether they follow rest, what movement does, and whether one or both legs are affected. Include caffeine, alcohol, exercise, sleep changes, pregnancy status, and every prescription or non-prescription medicine. Gentle stretching, warmth, and a short walk may provide temporary comfort.
A clinician may examine the legs and nerves and request blood tests such as iron studies or kidney-related tests when appropriate. Do not begin iron supplementation simply because the symptom is present; treatment should match a confirmed deficiency and medical advice.
When to seek care
Get urgent help for sudden weakness, severe new pain, one-sided swelling with warmth or redness, a cold or pale foot, or breathing difficulty. Seek prompt review for rapidly spreading numbness, an unnoticed wound, or symptoms that are escalating quickly. For recurring night symptoms without warning signs, book a routine appointment.
Questions for your doctor
Which features of my symptoms point towards restless legs syndrome or neuropathy? Should I have iron and kidney tests? Could a medicine or health condition be contributing? What changes would mean I should seek earlier review?

Questions people ask
Is iron deficiency the only cause?

Can medicines trigger restless-leg symptoms?
Some medicines may worsen or resemble these symptoms, so bring a complete medicine list to your review.
