Red spots on palms: what they may mean

Note whether the spots are flat or raised, smooth or scaly, itchy or painful, and whether they blanch when pressed. New soap, cleaning products, gloves, friction, heat, or sweating can irritate palm skin. Small bumps around hairs elsewhere may fit folliculitis, while palm lesions need a broader examination because palms have thick skin and few hair follicles. A clinician may ask about new medicines, exposures, fever, mouth sores, or spots on the soles.

Wash gently with lukewarm water and a fragrance-free cleanser, then apply a plain moisturiser after drying. Pause recently introduced products and protect your hands from detergents with suitable gloves. Do not scratch, pop, or use a strong steroid or acid product without advice; these can obscure the pattern or worsen irritation. Take a clear photograph and record when the spots began and whether they spread.

Seek urgent medical help if palm spots appear with breathing difficulty, facial swelling, faintness, widespread blistering, severe pain, or a high fever. Arrange prompt assessment for rapidly spreading redness, pus, marked swelling, purple or bruise-like spots, or sores in the mouth or eyes. A persistent or recurring rash also deserves a dermatology review.

Ask what features distinguish contact irritation from infection, whether any medicine or product should be stopped, and whether the soles, nails, mouth, or other skin areas should be checked.
