Red spots on the feet: what to check

Spots between the toes with scaling, cracking, or itch can occur with a moisture-related fungal rash. Redness where shoes or straps rub may be irritation or a pressure injury. Small bumps around hairs can represent folliculitis, while spots on the soles may have different causes because that skin is thicker and has no hair follicles. A new rash after a product, sock, or plant contact can reflect sensitivity.

Check both feet and look for swelling, warmth, tenderness, drainage, peeling, blisters, or colour change that does not fade with pressure. Do not diagnose from colour alone.

Wash gently, dry carefully between the toes, and change damp socks. Wear roomy, breathable footwear and avoid the suspected rubbing source. Do not share towels or footwear, scratch, or use strong acids on broken skin. A pharmacist or clinician can advise on an appropriate treatment when the pattern is clear.

Seek medical advice promptly for spreading redness, warmth, swelling, increasing pain, pus, fever, or difficulty walking. Get urgent help for a rapidly worsening foot, red streaks up the leg, severe swelling, numbness, or a darkening area. People with diabetes, poor circulation, or reduced sensation should arrange assessment early for new foot changes.

Ask whether the rash fits irritation, folliculitis, infection, or another condition; whether a swab or examination is needed; and which footwear or skin products to avoid.
