Rectal pain during ovulation: what to know

Ovulation can cause one-sided lower abdominal or pelvic discomfort, and pain may be felt toward the back or rectum because pelvic organs share nearby nerves. A short episode that follows a similar mid-cycle pattern may fit ovulation pain, also called mittelschmerz.

Rectal pain can also occur with bowel irritation, constipation, haemorrhoids, pelvic floor muscle spasm, or endometriosis. Endometriosis commonly presents with pelvic pain that may worsen around menstruation or bowel movements. Ovarian cysts can present with pelvic pressure or pain, especially if a cyst enlarges or causes sudden symptoms.

Record when the pain starts, how long it lasts, whether it follows your cycle, and whether bowel movements, sex, bleeding, fever, or urinary symptoms occur with it. Gentle heat, rest, fluids, and avoiding straining may help while you monitor a mild episode.

Arrange a clinical review if the pain keeps returning, interferes with daily activities, or is new for you. An examination may help distinguish anal causes from pelvic causes; your clinician may discuss pregnancy testing or imaging when the pattern warrants it.

Seek urgent care for sudden severe pelvic or rectal pain, fainting, heavy vaginal bleeding, fever, repeated vomiting, a rigid or swollen abdomen, or pain during a possible pregnancy. These features can signal a problem that needs prompt examination, including bleeding, infection, or an ovarian complication.
