Rectal Bleeding After Sex

Blood from the anus may come from a small tear, irritated haemorrhoid, dryness, inflammation, or friction, especially after anal penetration or difficult bowel movements. It can be hard to tell whether blood came from the rectum, vagina, or urinary tract, so note where it appeared, its colour, and whether it was on tissue, a condom, or in the toilet.

Rectal bleeding after sex can also occur alongside infection or inflammation. If the bleeding is vaginal rather than rectal, conditions such as cervical dysplasia, cervical cancer, vaginal cancer, chlamydia, or menopause-related tissue changes may need consideration. A symptom does not identify one diagnosis by itself.

Pause penetration until bleeding and pain have settled. Avoid putting products into the rectum, keep the area gently clean, and prevent straining with fluids and fibre if constipation is present. Arrange a medical appointment if the bleeding recurs, follows vaginal sex without a clear rectal source, or comes with discharge, sores, pain, altered bowel habits, or a possible sexually transmitted infection. A clinician may examine the anus, vagina, and cervix and offer infection testing.

Seek urgent care for heavy or ongoing bleeding, dizziness, fainting, severe rectal or abdominal pain, black stools, fever, or weakness. Emergency help is needed if a sharp object or foreign body is retained, or if there is sudden severe pain after penetration. Do not delay assessment because bleeding happened during consensual sex; the amount, persistence, and accompanying symptoms guide urgency.
