Puffiness Behind the Knee: What to Know

The hollow behind the knee contains skin, soft tissue, blood vessels, nerves, and structures linked with the joint. A soft puffy patch can follow friction, a bite, contact irritation, or an allergic reaction. Itching or raised areas may occur with hives or angioedema. Red, hot, tender skin can indicate cellulitis. A deeper fullness with stiffness or difficulty bending the knee may need an examination of the joint and nearby tissues. The appearance alone cannot identify the cause, particularly when the swelling is new or only affects one leg.

Compare both sides without repeatedly pressing the area, and note when the puffiness started, whether it changes with activity, and whether a rash or injury preceded it. Keep the skin clean, avoid scratching, and pause any new cream or detergent that touched the area. A cool compress can ease mild itchy swelling. Avoid tight clothing over the knee. Arrange a clinical review if it persists, returns, limits movement, or is accompanied by pain, warmth, or redness.

Seek emergency help for sudden leg swelling with chest pain, breathlessness, faintness, or coughing blood. Obtain urgent assessment for rapidly increasing swelling, severe pain, marked warmth or redness, fever, numbness, a cold foot, or inability to walk or bend the knee. Emergency help is also needed for swelling with throat or tongue symptoms. These warning signs can indicate an infection, a circulation problem, or a serious allergic reaction and should not be watched at home.

Ask whether the swelling seems superficial or deeper, whether the knee needs imaging or another examination, and what changes should prompt urgent care. Tell the clinician about injury, recent travel or immobility, new medicines, insect bites, skin products, fever, itching, and any swelling elsewhere.
