Psoriatic arthritis in the big toe: what to know

Short answer
Psoriatic arthritis can cause pain, swelling and stiffness in the big toe, sometimes with nail or skin changes.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common toe clues
- Swelling, warmth, stiffness, pain with walking, or nail changes.
- Assessment
- A clinician reviews the symptom pattern and examines the toe, skin and nails.
What it can feel like
Inflammation may affect the toe joint or the tissues where tendons attach. The toe can look puffy, feel warm or hurt when you push off while walking. Morning stiffness may ease as you move, but symptoms can return after rest. A nail that becomes pitted, thickened or partly separated can provide a useful clue, although toe pain also has many other possible causes.
A clinician considers the pattern of symptoms, skin and nail findings, examination results and sometimes imaging or blood tests. A painful big toe alone cannot confirm psoriatic arthritis.
Practical next steps
Arrange an assessment if the swelling or stiffness persists, keeps returning, or affects walking. Note which joints hurt, whether symptoms are worse after rest, and any psoriasis or nail changes. Shoes with a wide toe box can reduce pressure; pacing activity and using brief rest periods may also help.
Treatment is individual. A clinician may discuss anti-inflammatory medication, other medicines that control inflammatory arthritis, or physiotherapy to maintain movement and improve foot function. Do not start, stop or combine medicines without checking how they fit your health history.
When to seek care
Seek prompt medical advice for a suddenly swollen, hot and very painful toe, especially if you feel feverish or unwell, because infection and crystal arthritis need different treatment. Seek urgent help after an injury if the toe looks deformed, becomes numb, or cannot bear weight. New eye pain or blurred vision also needs same-day medical assessment.
Questions for your doctor
Could the toe symptoms reflect inflammatory arthritis, gout, an injury or another problem? Do my nails or skin provide clues? Would imaging or blood tests help? Which footwear, exercises and medicines are suitable for me? How should I monitor swelling and stiffness?

Questions people ask
Can psoriatic arthritis affect only the big toe?
It can affect one toe or several joints, but one painful toe still needs assessment because other conditions can look similar.
Is a swollen big toe always psoriatic arthritis?
No. Injury, gout, infection and other joint problems can also cause swelling, so the direction of diagnosis should come from an examination.
