Psoriasis in toe nails: signs and care

How the structures involved in Psoriasis differ from normal

Short answer

Psoriasis in toe nails results from inflammation in the nail growth area or nail bed and can resemble a fungal infection.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Typical toenail changes

Toenail psoriasis may produce pits, ridges, thickening, colour changes, or separation from the tissue below. Debris can collect under a lifted or thickened nail, making infection seem likely. Pressure from shoes can worsen tenderness and repeated injury can add another cause of nail damage. The pattern may involve more than one toe, but this is not a reliable way to diagnose it. A clinician may also look for psoriasis elsewhere, including the scalp or skin folds. See the main [Psoriasis](/conditions/psoriasis/) guide for broader context.

Care for affected toes

Choose shoes with enough room for the toes and change damp socks promptly. Trim nails straight across without digging into the corners, and do not pull away a separated nail. Avoid salon procedures that cut the cuticle or aggressively clean beneath the plate. A dermatologist can distinguish psoriasis from fungal disease and may recommend a nail sample before treatment. Tell the clinician about pain, previous injury, athlete’s foot, and any skin rash. Scaling or peeling between the toes may be relevant; read about [Scaly Skin](/symptoms/skin-scaling/) and [Peeling Skin](/symptoms/skin-peeling/).

When review is needed

Arrange care soon if the toe becomes red, hot, swollen, or drains fluid, or if walking becomes painful. Seek assessment for a dark streak, sudden separation, or a change limited to one nail that keeps progressing. New morning stiffness, swollen toes, or heel pain should also be reported because psoriasis can involve joints.

Useful questions

Does the pattern fit psoriasis or infection? Should the nail be tested? How can I protect it inside shoes? What symptoms would suggest joint involvement? How long should I use the recommended treatment before returning for review?

Questions people ask

Can toenail psoriasis be mistaken for fungus?

Yes. Thickening, colour change, and debris can occur with either condition, so examination or testing may be needed.

Should I remove a lifted toenail?

Do not pull it off; protect it and ask a clinician how to trim it safely.
