Proptosis and hyperthyroidism: what should you know?

Proptosis means the eye appears pushed forward. In thyroid eye disease, inflammation and swelling in tissues around the eye can affect the eyelids, eye muscles, and the space behind the eye. Symptoms may include a staring appearance, lid retraction, watering, dryness, light sensitivity, double vision, or difficulty closing the eyelids. Hyperthyroidism can also cause palpitations, excessive sweating, heat intolerance, diarrhoea, anxiety, or difficulty sleeping, but eye symptoms can need separate assessment even when thyroid symptoms are mild.

Arrange medical review for new eye prominence or a change in vision. Bring a list of medicines and describe whether symptoms affect one or both eyes, vary through the day, or worsen with movement. Lubricating eye drops may ease dryness when recommended by a clinician; sunglasses can reduce glare. Avoid smoking and second-hand smoke, which can aggravate thyroid eye disease. Do not change thyroid treatment without the prescriber’s guidance. An eye specialist may check eye movement, eyelid closure, vision, and colour vision.

Seek urgent eye care for reduced or blurred vision, loss of colour vision, a new field-of-vision gap, severe eye pain, marked swelling, inability to close the eye, or rapidly worsening proptosis. A severe headache, confusion, or weakness also needs emergency assessment. Prompt evaluation protects the surface of the eye and checks whether pressure or swollen tissues are affecting the optic nerve.
