Short answer

Promethazine can worsen restless-leg symptoms in some people, so discuss alternatives and possible causes with a clinician.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Check iron status before taking supplements

What this may mean

Restless legs causes an urge to move, often with crawling, pulling or uncomfortable sensations that become more noticeable while resting and may disturb sleep. Promethazine is an antihistamine used for problems such as nausea, allergy symptoms or sleep. Its sedating antihistamine action can make restless legs more troublesome for some people; feeling sleepy does not necessarily settle the underlying urge to move.

Symptoms can also be associated with low iron stores, kidney disease or nerve problems. The pattern matters: note when symptoms began, whether they follow each dose, and whether movement relieves them.

What to do next

Do not stop a prescribed medicine without advice. Contact the prescriber or pharmacist and explain the timing of your symptoms. They can review promethazine, other medicines, sleep, and possible contributors such as Iron Deficiency Anaemia, Chronic Kidney Disease or Peripheral Neuropathy.

Until you receive advice, keep a regular sleep routine, use gentle leg movement or stretching in the evening, and avoid taking extra sedating medicines. Do not start iron tablets simply because your legs feel restless; testing helps determine whether Iron Supplementation is appropriate.

When to seek care

Arrange medical advice if symptoms repeatedly disturb sleep, begin after a medicine change, or come with weakness, numbness, swelling or severe pain. Seek urgent help for sudden one-sided leg swelling and pain, chest pain, severe breathlessness, fainting, or a severe reaction such as facial swelling or trouble breathing.

Questions for your doctor

Could promethazine be contributing to my symptoms? Should the dose or medicine be changed? Do I need iron or kidney-function tests? What non-sedating option could help the problem for which promethazine was prescribed?

Questions people ask

Should I stop promethazine if my legs feel restless?

Speak with the prescriber or pharmacist first; they can advise whether a change is suitable and review other causes.

Can iron help restless legs?

Iron may help when testing shows low iron stores, but supplements are not suitable for everyone and should be guided by a clinician.
