Pregnancy puffiness: what to know

Short answer
Pregnancy puffiness can reflect normal fluid changes, especially later in the day, but sudden, marked, one-sided, or painful swelling needs prompt medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Mild swelling in both feet or ankles that increases later in the day
- Check promptly
- Sudden face or hand swelling, or one-sided painful swelling
- Emergency sign
- Breathing difficulty or throat and tongue swelling
How to read the pattern
Fluid can collect in the feet, ankles, hands, or face as pregnancy progresses. Warm weather, standing for long periods, and tight footwear can make it more noticeable. Puffiness that is soft, mild, and similar on both sides may be less concerning, but appearance alone cannot rule out a medical problem. Swelling with itchy welts or lip and eyelid enlargement can occur with hives or angioedema. Red, hot, tender skin suggests an infection such as cellulitis and needs assessment.
Supportive steps
Rest with your feet raised, change position regularly, avoid standing still for long periods, and wear comfortable shoes and non-restrictive clothing. Gentle movement approved by your maternity clinician may support comfort. Stay hydrated and follow your antenatal team's advice about salt and activity rather than making major dietary changes yourself. Do not use water tablets, compression garments, or new supplements for swelling without checking first, particularly if swelling is new or increasing.
When to seek urgent care
Contact your maternity team promptly for sudden swelling of the face or hands, a severe headache, visual changes, pain beneath the ribs, shortness of breath, or feeling unusually unwell. Same-day assessment is important for one-sided leg swelling, warmth, redness, or pain. Emergency help is needed for breathing difficulty, throat or tongue swelling, chest pain, faintness, or rapidly worsening swelling.
Questions for your doctor
Ask whether your swelling pattern needs blood pressure or urine checks, whether a skin condition is contributing, and which movement or support measures are suitable for you.
