Can preeclampsia occur without hypertension?

A normal blood-pressure reading does not explain every symptom or test result in pregnancy. Clinicians interpret readings over time with urine protein, blood tests and symptoms. They may also assess the baby’s growth, movements and placental function. A single normal result can be reassuring while still requiring follow-up if other findings are concerning.

Headache, visual disturbance, pain beneath the ribs, sudden swelling, nausea, vomiting or breathlessness should be described clearly. These symptoms can have causes unrelated to preeclampsia, so they do not diagnose it by themselves.

Tell your maternity team about new, persistent or worsening symptoms even if your home reading is within the advised range. Use the measurement technique you were shown and record the time, reading and symptoms. Attend urine, blood-pressure and blood-test appointments, because changes may become clearer when results are compared.

Seek urgent help for severe headache, new vision loss or blurring, severe upper-abdominal pain, chest pain, difficulty breathing, confusion, fainting or a seizure. Contact the maternity service promptly for reduced fetal movements or a marked change in how you feel. Do not wait for the next appointment when symptoms are severe.

Ask which findings are being followed, whether urine and blood tests are needed, how to measure at home, when to repeat checks, and which symptoms should take you directly to urgent care.
