What should you know about postpartum swelling after c section?

Swelling after a caesarean section is often related to fluid shifts after pregnancy and treatment, but sudden or one-sided swelling needs medical assessment. The feet and ankles are common places to notice it, and swelling can take time to settle.

During pregnancy, the body holds extra fluid. Intravenous fluids, hormonal changes and reduced movement around surgery can make fluid collect in the lower legs after birth. Swelling may affect both sides and can be more noticeable later in the day.

Swelling is not always a normal recovery symptom. One-sided leg swelling, warmth or pain can indicate a blood clot. Swelling with a severe headache, visual changes, upper abdominal pain or breathlessness can be associated with pre-eclampsia or another urgent problem.

Walk gently as advised by your surgical team, move your ankles while resting and raise your legs when comfortable. Drink normally, avoid tight clothing around swollen areas and follow wound-care instructions. Do not take water tablets or other medicines for swelling unless prescribed.

Postnatal physiotherapy can support safe movement, abdominal recovery and circulation after a caesarean section. Tell your clinician if swelling is increasing, remains marked or is accompanied by reduced urine, pain or difficulty moving.

Seek emergency help for sudden breathlessness, chest pain, coughing blood, fainting or a fast heartbeat. These can be signs of a blood clot that has travelled to the lungs. Seek prompt assessment for one swollen, painful, red or warm leg.

Contact your maternity team urgently for severe headache, vision changes, pain under the ribs, rapidly increasing swelling of the face or hands, fever, worsening wound redness or discharge. Do not wait for swelling to disappear on its own when these symptoms occur.

Ask whether the pattern of swelling is expected, how much walking is suitable, whether you need a blood-pressure or clot assessment, how to protect your incision during movement and which changes should trigger an emergency call.
