Postcoital and Contact Bleeding: What to Know

Postcoital bleeding is bleeding noticed after vaginal sex. Contact bleeding can also occur when the cervix or vaginal tissue is touched during an examination or another activity. A small amount may follow friction, especially when lubrication is low, or a minor surface injury.

Other possibilities include inflammation or a sexually transmitted infection such as chlamydia, changes related to menopause, or a cervical growth or cell change. Cervical dysplasia and cervical cancer can present with bleeding after sex, although this symptom does not establish either diagnosis. Vaginal cancer is another uncommon condition that may need consideration when bleeding persists or occurs with other concerning symptoms.

Note when the bleeding happens, how long it lasts, how much you see, and whether you also have pain, unusual discharge, itching, sores, or bleeding between periods. Avoid sex until discomfort or active bleeding has settled, and use lubrication when you resume if dryness or friction may be contributing.

Arrange a clinical assessment if the bleeding happens again, has no clear explanation, or follows sex with a new or untested partner. A clinician may ask about your menstrual pattern, contraception, pregnancy possibility, medicines, and screening history. They may examine the vagina and cervix and offer infection testing or other tests based on your findings.

Seek urgent medical help for heavy or continuing bleeding, severe or worsening pelvic pain, faintness, marked weakness, fever, or a possible pregnancy with pain or bleeding. Prompt assessment is also appropriate if bleeding follows sexual assault or an injury.

Book a timely appointment for repeated postcoital bleeding, bleeding after menopause, bleeding between periods, or a change in vaginal discharge. Bring details of the episodes and any recent test results. If a cervical abnormality is found, the clinician can explain whether monitoring, further examination, or a procedure such as cone biopsy is appropriate.
