What should you know about ponytail headache?

Pulling the hair can place pressure on sensitive nerves and tissues in the scalp, forehead, or around the ears. Pain may begin while the hairstyle is in place or shortly after it is removed, and tenderness can remain for a while. A headache that occurs with a blocked or runny nose may instead accompany allergic rhinitis, a common cold, or sinus disease. Head pain can also occur with dehydration, heat exhaustion, anxiety, or an infection, so timing and associated symptoms matter.

Loosen or remove the hairstyle, vary where the hair is tied, and avoid sleeping in a tight arrangement. Gentle scalp relaxation, fluids, rest, and a cool environment may help. If you use an over-the-counter pain reliever, follow its label and check with a pharmacist if you have a medical condition, take other medicines, or are pregnant. Keep a brief record of triggers, duration, and associated symptoms if headaches recur.

Seek emergency care for a sudden explosive headache, new weakness or numbness, trouble speaking, fainting, confusion, seizure, stiff neck with fever, or new vision loss. Prompt review is also needed after a head injury, with persistent vomiting, or when a new headache keeps returning despite removing the hairstyle. Fever, marked dehydration, or breathing difficulty should be assessed for their own warning signs.
