PND and heart failure: what to know

When you lie down, fluid that has collected in the legs can move back into the circulation. If the heart cannot handle the increased volume effectively, pressure may build in the lung blood vessels and make breathing difficult. A person may wake gasping and need to sit or stand to feel better. Heart failure can also cause daytime breathlessness, tiredness, a cough, reduced exercise tolerance, or swollen ankles. Other heart and lung problems can cause similar symptoms, so PND is not a diagnosis by itself.

Contact a clinician promptly if this symptom is new or recurring. Note when it happens, how long recovery takes, whether extra pillows help, and any weight or ankle-swelling change. Continue prescribed medicines unless a clinician tells you otherwise, and bring a medicine list to review. Assessment may include blood pressure, heart rhythm, chest examination, an ECG, blood tests, or imaging. A Chronic Disease Review can help monitor an established diagnosis.

Call emergency services for severe breathlessness at rest, blue lips, fainting, confusion, chest pressure, or breathlessness with a cold sweat. Sit upright while waiting and do not drive yourself if severely unwell. Prompt review is also needed for rapidly increasing ankle swelling, worsening cough, palpitations, or a clear decline in usual activity.
