Plastic surgery for scar removal: what to know

A flat pale line, an indented acne mark and a thick itchy keloid are different problems. A keloid grows beyond the original injury and can return after it is cut out, so surgery may be combined with another treatment. A scar that pulls across a joint or changes facial movement may need functional correction rather than cosmetic smoothing alone.

Options include steroid injection into the skin for selected raised scars, pulsed dye laser treatment for some red scars, fractional laser treatment for texture, cryotherapy for selected lesions and surgical excision or keloid removal surgery. The clinician also checks whether a new lump is truly scar tissue.

Show the clinician how the mark has changed and explain itching, tenderness, growth, tightness or movement problems. Mention acne history, previous procedures, darker marks after inflammation, and any tendency to form keloids. Do not pick or inject a changing lump yourself. Protect healing skin from sunlight and use the dressing or scar-care plan provided after treatment.

Arrange a clinical review for a scar-like lump that is enlarging, bleeding, ulcerated, painful or changing in colour. After treatment, report spreading redness, warmth, pus, fever, wound separation or rapidly worsening pain. Seek urgent help for uncontrolled bleeding, facial swelling affecting breathing or a sudden loss of movement.
