Short answer

A pertussis-related cough can arrive in repeated bouts and needs medical assessment when it is severe, persistent, or affects breathing.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Pattern
Repeated coughing bouts may be followed by a gasp or vomiting.
Assessment
A clinician can distinguish pertussis from other causes of prolonged cough.

What a pertussis cough can feel like

Pertussis, also called whooping cough, may begin like a mild cold before the cough becomes forceful or occurs in clusters. A person may gasp after a bout, vomit from coughing, feel exhausted, or notice soreness in the chest and abdominal muscles. The characteristic whoop is not present in every person, particularly adults. A cough may continue after the infection itself has started to settle. Other causes of cough include acid reflux, acute bronchitis, allergic rhinitis, asthma, bronchiectasis, bronchiolitis, chronic cough, and the common cold.

Practical steps

Arrange a clinical review if pertussis is suspected, especially after close contact with someone diagnosed with it or when coughing bouts are worsening. Describe when the cough began, whether it comes in spells, and whether you have retching, vomiting, fever, or breathing difficulty. Rest between bouts, sip fluids, and avoid smoke or other airway irritants. Cover coughs, wash hands, and limit close contact with babies and people who may become seriously ill until a clinician explains when normal contact is appropriate. Do not start leftover medicines or a cough suppressant without advice.

When to seek urgent help

Seek urgent help for struggling or paused breathing, blue or grey lips, severe chest pain, fainting, confusion, or inability to keep fluids down. Get prompt medical advice when coughing prevents sleep or eating, causes repeated vomiting, or is not easing. Babies, older adults, pregnant people, and anyone with a lung condition deserve a lower threshold for assessment because coughing illness can place extra strain on breathing.

Questions to take to an appointment

Could this pattern fit pertussis or another cause? Do I need testing or treatment? How should I protect household members? When can I return to work, school, or childcare? Which warning signs should make me seek urgent help?

Questions people ask

Can pertussis occur without a whoop?

Yes. Some people, especially adults, have prolonged coughing bouts without the classic whooping sound.

How long can the cough last?

The cough can outlast the early infection, so ongoing symptoms should be discussed with a clinician.
