What should you know about persistent pulmonary hypertension?

The blood vessels between the heart and lungs may become narrowed, stiff, or affected by another problem. The right side of the heart then has to work harder to move blood through the lungs. Symptoms can include breathlessness with activity, tiredness, dizziness, fainting, chest pressure, a fast heartbeat, ankle swelling, or bluish lips. Some people have few symptoms early on, so persistent breathlessness should not be explained away without assessment.

Possible associated conditions include Heart Failure, Chronic Kidney Disease, Obstructive Sleep Apnoea, Obesity, or other heart and lung disorders. Hypertension elsewhere in the body is a different problem, although both conditions involve pressure and can coexist. Tests may assess oxygen levels, heart function, lung function, and blood flow.

Arrange a medical review for ongoing breathlessness, reduced exercise tolerance, chest symptoms, or unexplained swelling. Keep a list of medicines and diagnoses, record activity-related symptoms, and follow the plan from your clinician. Avoid smoking and ask before strenuous exercise, high-altitude travel, or pregnancy planning because these may require individual guidance. Take prescribed treatment consistently and do not change it because symptoms briefly improve. A clinician may coordinate heart, lung, sleep, or kidney assessment.

Seek emergency help for severe or rapidly worsening breathlessness, fainting, chest pain or pressure, blue or grey lips, confusion, coughing blood, or a new fast or irregular heartbeat with weakness. Urgent review is also needed for sudden one-sided weakness, facial drooping, trouble speaking, or loss of balance, which can indicate Stroke or Transient Ischaemic Attack. Do not drive yourself if you are faint, severely breathless, or confused.
