Perioral dermatitis on the forehead: what to know

Perioral dermatitis is named for its usual area around the mouth, yet related bumps may appear around the nose, eyes or forehead. Small, fairly uniform bumps with redness, dryness, tightness or burning are useful clues. A narrow area of spared skin next to the lips can also support the diagnosis, but appearance alone cannot confirm it.

Forehead spots may instead reflect blocked pores, hair products, sweat, an infected follicle or another dermatitis. The timing of new skincare, makeup, hats, bangs or steroid use can help a clinician separate these possibilities.

Use lukewarm water and a gentle cleanser, then pat dry. Pause new or heavily fragranced products and keep hair oils, styling products and thick occlusive creams off the forehead. Avoid squeezing the bumps, which can prolong inflammation and leave marks.

A clinician may recommend topical antibiotic therapy or oral antibiotics when the overall pattern fits perioral dermatitis. Topical corticosteroids are not a routine self-treatment for this condition and may aggravate it, so discuss any current steroid use before changing it.

Book a review if forehead bumps last beyond a short period of gentle care, spread toward the eyes or mouth, or keep recurring. Get prompt help for eye pain, vision change, facial swelling, fever, rapidly spreading redness or painful drainage.
