Period-type pain in the middle of your cycle

Ovulation pain, also called mittelschmerz, may feel like a cramp or sharp ache on one side of the lower abdomen. It can occur around the middle of the cycle and settle without other illness. Endometriosis can present with pelvic pain that follows a cycle, while an ovarian cyst can cause pressure or one-sided discomfort. Pain alone cannot identify which explanation applies, and a change from your usual pattern deserves attention.

Note when the pain starts, which side it affects, how long it lasts, and whether bleeding, discharge, bowel symptoms, urinary symptoms, or nausea occur. Rest, gentle heat, and an appropriate over-the-counter pain medicine may help if you can take it safely. Arrange a clinical review if pain recurs, interferes with activities, or does not settle. Pregnancy possibility should be mentioned because it changes the assessment.

Seek urgent help for sudden severe pelvic pain, fainting, shoulder-tip pain, heavy bleeding, fever, persistent vomiting, or pain with a positive pregnancy test. Prompt review is also sensible when pain steadily worsens or comes with a swollen abdomen or marked tenderness.

Does the timing fit ovulation pain? Could endometriosis or an ovarian cyst explain the pattern? Would an examination, pregnancy test, or ultrasound help?
