Period after miscarriage: timing and warning signs

Bleeding immediately after a miscarriage is part of the pregnancy loss and is different from a menstrual period. The menstrual cycle resumes as hormone levels settle and ovulation returns. The first period may be heavier, lighter, longer, or more crampy than usual. Pregnancy can occur before the first period, so discuss contraception if you are not trying to conceive.

Ongoing pregnancy symptoms, increasing pain, fever, unpleasant-smelling discharge or bleeding that does not ease can point to tissue remaining in the womb, infection or another problem. These symptoms need clinical assessment rather than waiting for a period to appear.

Keep a simple record of bleeding, pain and temperature if you feel unwell. Arrange follow-up with the clinician or service that treated you, especially if no period returns when you expected or symptoms continue. A transvaginal ultrasound may help assess the womb. Treatment choices, when needed, can include medical or surgical management, depending on findings and your preferences.

Seek urgent help for very heavy bleeding, faintness, severe or one-sided pain, fever, chills, or feeling suddenly unwell. Contact your maternity or gynaecology service promptly if bleeding becomes heavier after improving or discharge develops a strong unpleasant smell.
