Why a toe callus can hurt

A callus is a thicker layer of skin formed by repeated friction or load. It can become tender when footwear presses on it, when the thickened centre concentrates pressure, or when the skin cracks. A bent toe, bunion, stiff big-toe joint, or altered walking pattern can make one spot repeatedly rub. Hammer Toe, Bunions, or Hallux Rigidus may therefore contribute to the pressure pattern, but a callus does not by itself diagnose one of these conditions.

Redness, warmth, swelling, drainage, or pain out of proportion to the visible skin change may indicate irritation or another problem. Gout can cause a suddenly painful, hot, swollen toe and should not be treated as an ordinary callus without assessment.

Ways to reduce pressure

Wear shoes with a roomy toe box and soft, non-rubbing material. A cushioned pad can move pressure away from the callus, provided it does not make the shoe tighter. Keep the skin clean and moisturised, but do not cut, shave, or chemically remove the callus yourself, particularly if you have diabetes, poor circulation, or reduced sensation. A podiatry or medical assessment can safely examine the skin, toe alignment, footwear pressure, and walking pattern.
