Pain when ovulation occurs

Ovulation pain, sometimes called mittelschmerz, may feel like a cramp, twinge, or ache on one side of the lower abdomen. It can switch sides in different cycles and may occur around the middle of a cycle. A pain diary can show whether the timing is consistent. Endometriosis can cause pelvic pain that follows the cycle but may also continue outside ovulation. Ovarian Cysts can cause pressure or pain, particularly when a cyst enlarges, leaks, or twists.

Use gentle heat, rest, and comfortable movement if these help. Note the side, intensity, duration, bleeding, discharge, fever, urinary symptoms, and bowel changes. Seek an appointment if pain keeps returning, interferes with normal activities, or is not clearly linked with ovulation. A clinician may discuss pregnancy testing, examination, or imaging based on your symptoms.

Get emergency help for sudden severe pelvic pain, fainting, weakness, shoulder-tip pain, heavy bleeding, persistent vomiting, or a possible pregnancy with pain or bleeding. Fever, a rigid abdomen, or rapidly worsening pain also needs prompt assessment. These symptoms can require urgent treatment rather than home monitoring.

Ask how ovulation is being identified, whether the timing fits the symptom, whether a cyst or endometriosis needs evaluation, and which changes should prompt urgent review.
