Ovulation pain: why does it happen?

Ovulation pain can happen when an ovary releases an egg and may cause brief, one-sided pelvic discomfort. It is also called mittelschmerz or mid-cycle pain. The sensation may be a twinge, cramp, ache, or sharper feeling, and it can switch sides from one cycle to another.

What the pain may mean

The growing follicle can stretch the ovary before release, and fluid or a small amount of blood from the follicle can irritate nearby tissue afterward. Pain that follows a familiar mid-cycle pattern and settles on its own is often less concerning than pain that is new, persistent, or steadily worsening. Other pelvic conditions can feel similar, including Endometriosis and Ovarian Cysts, so the timing alone cannot confirm the cause.

What you can do

Note when the pain starts, which side it affects, how long it lasts, and whether bleeding, discharge, fever, bowel symptoms, or urinary symptoms occur with it. Gentle heat on the lower abdomen, rest, and ordinary movement may help. If you use an over-the-counter pain medicine, follow its label and check with a pharmacist or clinician if you are pregnant, have stomach or kidney problems, take blood thinners, or have another reason to avoid it.

When to seek care

Arrange medical advice if pain repeatedly interrupts daily activities, lasts beyond the expected mid-cycle window, returns more often, or does not improve with simple measures. Seek urgent help for sudden severe pain, fainting, marked dizziness, heavy bleeding, fever, repeated vomiting, a swollen abdomen, or pain during a possible pregnancy. These features need prompt assessment because several conditions can cause them.
