Timing and symptoms

The exact day varies because cycles do not all follow the same schedule. You may notice a one-sided ache or cramp between periods, sometimes with a small amount of spotting or clear, slippery cervical mucus. The discomfort may last briefly or continue for part of the day, then fade.

A calendar estimate is only a guide. Stress, illness, breastfeeding, hormonal contraception, and naturally changing cycles can shift ovulation or make it harder to identify. Endometriosis and ovarian cysts can also produce pain near the same part of the cycle, so a repeated pattern does not establish the cause.

How to track it

Mark period days and pain days in an app or calendar. Note the pain's side, strength, duration, bleeding, discharge, and any effect on urination or movement. If you are trying to understand fertile timing, combine this record with cervical mucus changes or an ovulation test rather than relying on pain alone. Heat and rest may soothe mild symptoms.

When the timing is not reassuring

Contact a clinician if pain occurs at unexpected times, lasts longer than your usual pattern, returns more intensely, or comes with fever, vomiting, unusual discharge, or bleeding. Seek urgent care for sudden severe pelvic pain, faintness, or pain when pregnancy is possible. A clinician can distinguish cycle-related discomfort from causes needing treatment.
