When does ovulation pain occur?

The pain, also called mittelschmerz, may happen when an ovarian follicle expands or releases fluid as an egg leaves the ovary. A cycle is not always the same length, so the middle of the cycle is a guide rather than a fixed calendar date. Some people feel the sensation before ovulation, while others notice it afterward.

It may be a one-sided ache, cramp, or brief sharper twinge. The side can change from one cycle to another. Pain that occurs at a different point, persists well beyond the expected window, or grows more intense should not be assumed to be ovulation pain.
