Ovulation Pain on the Left Side Only

Ovulation pain, also called mittelschmerz, is often felt low in the abdomen or pelvis on one side. The side may vary from cycle to cycle, and some people notice it on the same side repeatedly. It may feel like a twinge, cramp, pressure or ache and can last briefly or continue for part of the day. Mild spotting or a change in vaginal discharge can occur around ovulation.

Left-sided pain does not by itself identify the cause. Bowel irritation, urinary problems, period-related cramps, endometriosis or an ovarian cyst can produce similar sensations. Endometriosis can cause pelvic pain that follows a cycle, while an ovarian cyst may cause pressure or one-sided discomfort. Pain that is new, steadily worsening or unrelated to the middle of the cycle deserves a clinical review.

Record the first day of your period, the day the pain starts, its location, how long it lasts and any bleeding, discharge, nausea or urinary symptoms. A warm pack over the lower abdomen, rest and gentle movement may help mild discomfort. Use an over-the-counter pain medicine only if it is suitable for you and follow the label directions; a pharmacist can help if you are unsure.

Seek urgent medical help for sudden severe pelvic pain, fainting, marked weakness, repeated vomiting, fever, heavy bleeding or pain with a possible pregnancy. Arrange a medical appointment if the pain keeps returning, lasts beyond the expected mid-cycle window, interferes with daily activities or comes with pain during sex, unusual discharge or difficulty passing urine or stool.
