Short answer

Ovulation pain, also called mittelschmerz, may feel like a twinge, ache, or cramp on one side of the lower abdomen around the middle of a cycle. It can shift sides from cycle to cycle and may last a short time. The timing can suggest ovulation, but pain alone cannot confirm that an egg has been released.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What mid-cycle pain can mean

As an ovary releases an egg, the follicle and nearby pelvic tissues can cause local irritation. Some people also notice light spotting, vaginal discharge changes, or a pressure-like feeling. These symptoms are not a reliable way to diagnose pregnancy or another condition.

Pain that is new, increasingly strong, present outside the usual cycle window, or associated with painful periods may have another explanation, including endometriosis. An ovarian cyst can also cause pelvic discomfort, especially if the sensation differs from your usual pattern.

Ways to manage it

Note when the pain starts, which side it affects, how long it lasts, and whether bleeding, nausea, fever, or discharge occurs. A warm bath or heating pad may relax tense muscles. Rest and gentle movement can be useful; stop activity if it worsens the pain. Use an over-the-counter pain reliever only if it is suitable for you and follow its label directions. If you are trying to conceive, discuss pain medicine choices with a clinician or pharmacist.

When to seek medical care

Seek urgent care for sudden severe pelvic pain, faintness, marked weakness, heavy bleeding, fever, repeated vomiting, or pain with a possible pregnancy. Arrange a medical review when pain keeps returning, lasts longer than expected, interrupts daily activities, or does not improve with simple measures. A clinician may ask about your cycle and examine you before deciding whether testing or imaging is needed.

Questions to take to an appointment

Ask whether the timing and location fit ovulation pain, what other causes should be considered, and which warning signs should prompt urgent help. You can also ask whether a symptom diary, pelvic examination, pregnancy test, or ultrasound would clarify the cause.
