How long before ovulation can ovulation pain start?

Ovulation pain may begin shortly before ovulation, occur as the follicle releases an egg, or continue briefly afterward. It is often felt on one side of the lower abdomen and may switch sides in different cycles. Calendar timing alone cannot confirm exactly when ovulation happens.

The discomfort may come from the ovary stretching as a follicle grows, or from a small amount of fluid irritating nearby tissue after release. A sharp twinge, ache, or pressure can fit this pattern, especially when it follows a similar point in several cycles. Pain that is new, persistent, severe, or unrelated to your cycle may have another cause, including endometriosis or an ovarian cyst.

Record the first day of your period, when pain begins, which side hurts, and how long it stays. Gentle movement, rest, a warm pack over the lower abdomen, and fluids may help. Use a pain reliever only if it is suitable for you and follow the product directions. If you are trying to avoid pregnancy, do not use pain timing by itself as contraception.

Seek urgent care for sudden severe pelvic pain, fainting, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, or pain with a possible pregnancy. Arrange a clinical review if pain keeps returning outside the expected cycle, lasts longer than your usual pattern, interferes with daily activities, or comes with unusual discharge or pain during sex.

Ask whether your symptom pattern fits ovulation pain, whether a pregnancy test or pelvic examination is appropriate, and whether another cause should be checked. Bring your cycle and symptom notes, including medicines, bleeding changes, bowel or bladder symptoms, and any history of pelvic conditions.
