What should you know about osteitis pubis and ankylosing spondylitis?

Short answer
Osteitis pubis is irritation around the pubic joint, while ankylosing spondylitis is an inflammatory condition affecting the spine and linked joints.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Osteitis pubis
- Pain centred around the pubic joint or groin
- Ankylosing spondylitis
- Inflammatory stiffness often involving the spine and hips
- Helpful support
- Assessment-led exercise and mobility care
How the terms differ
Osteitis pubis may cause groin or lower pelvic pain, particularly after repeated loading, sudden increases in training, or stress around the abdominal and thigh muscles. Ankylosing spondylitis more often causes inflammatory back or buttock pain, stiffness after rest, and reduced spinal movement. The two names should not be treated as interchangeable.
Joint stiffness can occur with many conditions and does not by itself identify ankylosing spondylitis. Morning joint stiffness that improves with movement, recurring symptoms, or stiffness affecting the back and hips deserves a clinical assessment. A doctor may combine a physical examination with blood tests or imaging when appropriate.
Planning care
Temporarily reduce movements that reproduce pelvic or back pain, but avoid prolonged bed rest unless specifically advised. Gentle mobility within comfort can prevent unnecessary deconditioning. Physiotherapy may address posture, breathing, spinal mobility, pelvic control, and a gradual strengthening plan.
For confirmed inflammatory disease, a clinician may discuss anti-inflammatory medication and a longer-term monitoring plan. Exercise therapy can preserve movement, while hydrotherapy may offer a lower-impact way to practise mobility. Do not start, stop, or combine medication without checking how it fits your health history.
When to seek care
Book a medical review for persistent groin or back pain, repeated morning stiffness, or a noticeable loss of movement. Seek urgent care for new leg weakness, numbness around the groin, loss of bladder or bowel control, severe pain after an injury, fever, or a hot swollen joint. These signs need prompt assessment rather than home exercise alone.
Questions to take along
Ask which features point toward osteitis pubis, ankylosing spondylitis, or another cause, and whether imaging or blood tests are appropriate. Discuss safe exercises, medication precautions, and the symptoms that should bring forward your next review.

Questions people ask
Are osteitis pubis and ankylosing spondylitis the same condition?
No. Osteitis pubis affects the pubic joint region, whereas ankylosing spondylitis is an inflammatory condition involving the spine and related joints.
Can physiotherapy help with these problems?
A tailored programme can support mobility, strength, posture, and a gradual return to activity, but the exercises should match the diagnosed cause.
