Orthostatic hypertension side effects: what you should know

A rise in blood pressure after standing can feel like a pressure-type headache, awareness of the heartbeat, flushing, lightheadedness, or unsteadiness. Symptoms may also come from the underlying hypertension, a medicine effect, dehydration, anxiety, or a separate heart or neurological problem. The symptom itself does not identify the cause.

Record what happens when you stand, how soon it starts, how long it lasts, and whether it settles after sitting. Include your blood-pressure reading, pulse if available, recent meals and fluids, and the name and timing of medicines. This gives the clinician useful context for distinguishing treatment effects from the blood-pressure pattern.

Sit down if dizziness or weakness begins, avoid driving while affected, and stand slowly from a chair or bed. Measure blood pressure only as advised, using consistent posture and technique. Do not stop, double, or rearrange prescribed medicines without speaking to the prescriber, even if a symptom appears soon after a dose.

Book a review if symptoms repeat, disturb sleep, limit activity, or begin after a treatment change. Bring your medicine list and symptom record. Individual advice may be needed when hypertension coexists with heart failure, kidney disease, obesity, or obstructive sleep apnoea.

Call emergency services for chest pain or pressure, severe breathlessness, fainting, sudden severe headache, new weakness or numbness, facial drooping, trouble speaking, confusion, or sudden loss of vision. These symptoms may indicate angina, stroke, or another emergency. Do not wait for a home reading to decide whether those warning signs matter.
