Optical Cellulitis Symptoms: What to Know

Cellulitis is a bacterial infection affecting deeper layers of skin. Around the eye, infection may involve the eyelid and nearby skin, or it may extend into tissues within the eye socket. Early signs can be one-sided redness, warmth, swelling, tenderness, and a tight or heavy feeling in the eyelid. Fever, chills, tiredness, or worsening pain can occur when the body is responding to infection.

Pain with eye movement, reduced or blurred vision, double vision, trouble moving the eye, or an eye that appears pushed forward are more concerning than surface eyelid swelling. A recent sinus infection, skin injury, insect bite, or nearby skin infection can provide a route for bacteria. Athlete's Foot may also create cracks that allow bacteria to enter elsewhere on the skin.

Arrange a same-day medical assessment for new or rapidly worsening redness and swelling near the eye, especially with fever or significant tenderness. Do not squeeze the area, pierce a blister, or apply leftover antibiotics or steroid creams. Keep the skin clean, avoid contact lenses and eye makeup, and tell the clinician about recent sinus symptoms, bites, injuries, or medical conditions such as Type 2 Diabetes.

A clinician may examine the skin, eyelids, eye movements, vision, and general health. Treatment depends on how deep the infection appears and can require antibiotics, monitoring, or hospital care. Do not delay assessment while waiting to see whether swelling settles on its own.

Go to an emergency department promptly if swelling is rapidly increasing, the eye is painful to move, vision is blurred or reduced, the eye looks displaced or bulging, or you cannot move it normally. Seek emergency help for severe headache, vomiting, confusion, faintness, breathing difficulty, or feeling suddenly very unwell. These features can signal infection spreading beyond the skin, including Sepsis, and need immediate evaluation.

Do not drive if your vision is affected. If symptoms are milder but clearly worsening, contact an urgent medical service the same day rather than waiting for a routine appointment.

Ask whether the infection appears limited to the eyelid or may involve deeper tissues, which symptoms should prompt an emergency visit, and how soon you should be reviewed after starting treatment. Mention any allergy to antibiotics, recent sinus or skin problems, medicines you take, and changes in vision or eye movement.
