A vacuum-assisted vaginal birth uses a soft cup on the baby’s head to help complete delivery when extra assistance is needed. It is performed by a trained maternity team after checking the baby’s position, the cervix, and whether vaginal birth remains appropriate.

What vacuum assistance means

The cup is placed on the baby’s scalp during a contraction, and gentle traction follows your pushing effort. A vacuum may be considered when labour is taking longer than expected, you are too tired to keep pushing effectively, or the baby needs to be born sooner because of changes in the heart-rate pattern. The clinician explains the reason and checks that the alternative options are understood.

After birth, the baby may have temporary swelling or a mark where the cup was placed. Your maternity team examines the baby and monitors feeding, colour, breathing, and alertness. You may have stitches, soreness, or bruising around the vagina and perineum.

What you can do

During recovery, follow the discharge advice about keeping any stitches clean, managing discomfort, resting, and gradually returning to activity. Tell the team if passing urine is difficult or if pain is worsening rather than settling. Before leaving, ask how the baby’s scalp should look, which newborn changes need review, and who to contact outside clinic hours. Keep your postnatal appointments so healing, bleeding, mood, and the baby’s progress can be assessed.

When to seek care

Contact maternity services urgently for heavy or suddenly increasing bleeding, faintness, severe abdominal or perineal pain, fever, foul-smelling discharge, or a wound that is opening. Seek prompt assessment for a severe headache, vision changes, chest pain, breathlessness, or one-sided leg swelling. For the baby, seek urgent help for trouble breathing, unusual sleepiness, poor feeding, repeated vomiting, or a rapidly enlarging scalp swelling.

Ask why vacuum assistance is being recommended, what alternatives are suitable, and what signs would lead the team to change the birth plan. After delivery, ask how deep any tear is, how to care for it, when driving or exercise may be reasonable, and what follow-up is arranged. You can also ask whether induction, Caesarean Section, or another birth-care service is relevant to your circumstances.
