Normal delivery after a first caesarean

Your team reviews the type and location of the uterine incision, the operative report, the reason for the first caesarean, and any other uterine surgery. They also consider the placenta, fetal position, pregnancy complications, and how labour begins. A prior caesarean leaves a scar in the uterus, so labour needs a setting where maternal and fetal wellbeing can be monitored and an urgent caesarean can be arranged if needed. The decision is personal and may be revisited as pregnancy develops.

Bring the previous surgical notes if they are available and discuss your preferred birth route early. Ask how spontaneous labour and induction would be managed, because induction can change the plan and may require additional monitoring. Make sure you understand the reasons your team might recommend a repeat caesarean. Prepare for both possibilities, including practical arrangements for recovery, feeding support, and help at home.

Call your maternity unit for regular contractions, leaking fluid, vaginal bleeding, or reduced fetal movement. Seek emergency care for severe or constant abdominal pain, faintness, heavy bleeding, or sudden worsening during labour. Do not wait for a routine appointment if symptoms are concerning; the previous uterine scar makes prompt assessment especially important.

Ask what your previous incision means for labour, whether your records are complete, how induction would be handled, and which hospital resources are available. Discuss pain relief, continuous monitoring, and the circumstances that would lead to caesarean section.
