No ovulation pain: should you be concerned?

Ovulation pain, also called mittelschmerz, is one possible sensation around the middle of a cycle. It may vary in side, intensity, and timing, and it can disappear from one cycle to the next. Not feeling it cannot confirm that ovulation did not happen. Period timing, cervical mucus, basal temperature, LH testing, and clinical tests provide different kinds of information.

If cycles are irregular, the absence of pain may simply reflect a changed pattern. Endometriosis can cause pelvic pain, while ovarian cysts may cause pain or no symptoms; neither can be diagnosed from the presence or absence of ovulation pain alone.

Continue tracking the signs that matter to your goal. For conception, combine cycle observations rather than waiting for pain. For contraception, do not treat the absence of pain as evidence that pregnancy is impossible. A clinician can discuss blood tests or ultrasound if ovulation needs confirmation.

Arrange an appointment for repeated long or unpredictable cycles, difficulty conceiving, new pelvic symptoms, or other signs that your cycle has changed. Seek urgent care for severe sudden pain, fainting, fever, vomiting, heavy bleeding, or pain during a possible pregnancy.
